What to Expect After Submitting Your Form:

1. A member of our team will send you a request to join our online patient hub, Practice Better. This is where we do all of our interaction, education, and coaching for nutrition and lab services.
   
   Watch your email inbox for links to set-up your patient account and download the free app for your phone.
   
   
   

2. After you activate your account we will send you digital copies of our new patient paperwork in the portal, along with a Welcome Letter walking you through our process for staring as a new nutrition client.
